2021 PDX FSDO Annual Accident Report

2021 PDX FSDO Annual Accident Report
by Joseph P. Mollahan, FAASTeam Program Manager - Operations, Portland Flight Standards District Office

Joe Mollahan is an aviation safety professional who enlisted in the United States Army (Oregon National Guard) in 1982 as an Observation Helicopter Repairman. In 1984, Joe attended the U.S. Army Warrant Officer Rotary Wing Aviator Course, graduating in March of 1985 and appointed a Warrant Officer. During his military career, he worked as a Maintenance Test Pilot, a Standardization Instructor Pilot, and an Aviation Safety Officer.

Joe’s military flight experience is in TH-55A, OH-58, UH-1H, CH-47, U-21, C-12, and C-23 aircraft. Joe deployed to Iraq in 2005 as a Platoon Leader in Theater Airplane Company flying C-23 aircraft. Joe also deployed to Iraq in 2009 as the Battalion Standardization Pilot with oversight responsibility for C-12, C-23, UC-35, and UH-60 aircraft.

While flying for the military, Joe continued to add civil ratings to his pilot certificates. Joe holds an Airline Transport Pilot Certificate with an SD-3 type rating. Joe has Commercial Pilot privileges for Airplane Single Engine Land, Rotorcraft Helicopter, and Instrument Helicopter. Joe is a Certificated Flight Instructor with Airplane Multi-Engine Land, Single Engine Land, Rotorcraft Helicopter, Instrument Airplane and Helicopter.

Upon retirement after 25 years on active duty, Joe started his own business at the McMinnville Airport providing professional pilot services and flight instruction. Joe worked as a contract pilot-in-command for the National Aeronautics and Space Administration conducting environmental science data collection measuring naturally occurring carbon gas emissions in Alaska.

After a spirited discussion with an FAA Inspector, Joe applied for, and accepted, a position with the FAA as an Aviation Safety Inspector in March of 2014. Joe became an FAA Safety Team Program Manager in February of 2021. This position allows Joe to focus on the promotion of aviation safety within the general aviation community.

Joe owns a 1976 Cessna 177RG. He has flown to Oskosh for the EAA Airventure in 2017, 2018, 2019, and 2021 and many other locations in the United States.
